Child Medical Expense Subsidy: Eligibility & How to Apply [2026]
The child medical expense subsidy is a program where the municipality covers your child out-of-pocket medical costs (called nyuyoji iryohi josei or maru-nyu / maru-ko depending on the area). Children of foreign residents who are registered and enrolled in health insurance are also covered.
① Who is eligible
- A child registered as a resident in that municipality and enrolled in health insurance
- The covered age differs by municipality (preschool / through junior high / through the fiscal year they turn 18, etc.)
- Some municipalities have income limits or a small co-payment (a few hundred yen per visit)
② How you use it
Show the medical certificate (jukyusha-sho) issued after applying, together with the health insurance card, at clinics and pharmacies. Your counter payment is free or reduced (some areas pay first and refund later).
③ How to apply
Apply at your municipal office, soon after birth or moving in.
